Uses of Praziquantel

 

Praziquantel is primarily used for the treatment of several parasitic infections, including:

 

Schistosomiasis: Praziquantel drug is the choice for treating schistosomiasis, an infection caused by trematode worms of the genus Schistosoma. It is effective against all species responsible for this disease, including Schistosoma mansoni, Schistosoma haematobium, and Schistosoma japonicum.

 

Cysticercosis: This condition is caused by the larval stage of the tapeworm Taenia solium. Praziquantel is used to treat viable cysts in the central nervous system.

 

Other Trematode Infections: The medication is effective against various other fluke infections, such as Fasciola hepatica (liver fluke) and Clonorchis sinensis (Chinese liver fluke).

However, published in vitro studies suggest a potential lack of efficacy against migrating schistosomulae.

 

An interesting characteristic of praziquantel is its relative ineffectiveness against juvenile schistosomes. Although it is initially effective, its efficacy decreases over the first 3 to 4 weeks. Subsequently, effectiveness increases again, returning to full efficacy by 6 to 7 weeks.

 

Mechanism of Action

 

While the exact mechanism of action of praziquantel remains unclear, it is hypothesized to target the β subunits of voltage-gated Ca²⁺ channels, especially in Schistosoma mansoni and Schistosoma japonicum. This hypothesis is supported by findings that co-administration of calcium channel blockers, such as nicardipine and nifedipine, leads to a 50% resistance to praziquantel in Schistosoma mansoni.

 

Additionally, increased exposure of antigens on the surface of the worms has been observed, though limited research has been conducted to further elucidate the specific mechanisms underlying its action.
